The LMJC Website prevents us from posting this Addendum in Word format. If you need a copy to save for future use, email the Court Administrator and ask for a Word copy of the Child Support Addendum to be emailed.

All parts of this addendum must be completed. This addendum may be completed by the parties or by the Court. Do not delete paragraphs which do not apply.

Mental Health Evals (Reminder)

Once a Superior Court Judge has signed a mental evaluation for DBHDD (Department Of Behavioral Health And Developmental Disabilities), a copy of the signed order MUST be sent immediately to Amy Brown in Judge Arnt’s office.
